We have several ways to make an appointment:

By calling 919-233-0410, or you can visit the patient portal and request an appointment through the portal. We ask you to bring your insurance card, drug plan card, and driver’s license to your appointments for billing and administrative filings.

First Visit

Your first patient appointment will be 30 minutes and in order to maximize your time and the efficiency of your appointment, we ask you to come in 30 minutes prior to your appointment to complete the necessary paperwork. We ask you to bring your driver’s license, insurance card and drug card to assist us with administrative filings.

If you would like, you can print off the following forms and complete and bring to your first appointment. To assist with your appointment needs, we ask you to have your forms ready prior to your visit to aid in making your appointment more efficient and timelier.

After Hours or Emergency Care

  • If you feel you have a life-threatening emergency, call 911 and go to the nearest emergency room.
  • For non-urgent calls that cannot wait until the next business day, call 919-233-0410 and leave a message for a return call the next business day.
  • Prescription Refills: Refills are sent in during normal business hours. Please prepare ahead and make sure your prescription needs are met during our normal hours.
  • Controlled Medications our policy is to have a 48-hour notice when requesting refill. We do not refill controlled medications after normal business hours.

* If your insurance requires you to pay a copay, you will be asked to pay when you check in for your appointments. For any billing questions, you can contact our billing department.

Patient Portal

Our practice has a web-based patient portal through electronic health system. Please contact the front desk staff to assist you with setting up your access.

The portal has many options for you as a patient:

  • Request appointments
  • Request medication refills
  • Send a secure message to your provider
  • Review your lab and x-ray results
  • Review your medications and certain patient records
  • Pay your bill
  • Check in for your appointment
  • Complete patient information forms
